1. Shop at Monument’s locally-owned indepedent businesses between June 1 and August 31, 2010
2. Save your receipts in this envelope; it is your entry form.(Original receipts only will be accepted; all sales must have sales tax included and must bear the name of the business to be eligible. Original receipts cannot be returned.)
3. When your receipts total at least $350.00, including sales tax, write your name, address, phone number and email address on the back of this envelope.
4. Turn the envelope in to one of the participating businesses or to Monument Town Hall, Beacon Lite and Hwy 105 no later than September 3, 2010. Entries will be checked for eligibility of receipts and qualifying total.

One winner will be awarded the
